How to Schedule an Appointment.

  1. Schedule an appointment using the booking widget below.

  2. Submit intake form and sign paperwork. Required to confirm booking.

    Please be sure to include an accurate email and/or phone number.

  3. Once submitted, staff will review the completed documentation to determine if you are eligible for services with this practice.

    If you are an insurance patient, your health insurance information provided will be checked for eligibility.

  4. Once eligibility is determined, you will receive an email informing you of the eligibility decision and important information regarding signing up for the Spruce App.

    You will additionally receive a link to the patient portal and receive a text/email with the practice policies and consents that need to be completed prior to your visit.

    ******Practice Policies and Consents are due 24 hours after scheduling your appointment or your appointment with be cancelled and you will need to reschedule.******

    New Self-Pay Clients will not be charged for appointments until after the staff have determined eligibility. When you are confirmed eligible for telehealth with this practice, you will be charged for your visit.

Important Notes

  • We only treat Adults, ages 18 or older.

  • We do not prescribe controlled substances such as Xanax, Gabapentin, or Ambien, etc.

  • We are unable to accept patients with Medicaid or Medicare.

  • We are unable to care for actively pregnant clients at this time.

  • Practice policies and consents are due 24 hours after scheduling the appointment or your appointment will be cancelled, NO EXCEPTIONS.
